Fenugreek & Breastfeeding

Fenugreek seeds contain hormone precursors that increases milk supply. Scientists do not know for sure how this happens, but it is believed that fenugreek stimulates breast milk glands, as they are similar to sweat glands. Women all around the world today take fenugreek seeds in capsules or pills. The capsules or pills can be found at most vitamin and nutrition stores, some supermarkets and natural foods stores. Fenugreek can also be taken in tea form, although tea is believed to be less effective than the capsules.

Fenugreek is considered safe for nursing moms when used in moderation and is found on the U.S. FDA's (Food and Drug Administration) 'Generally Recognized As Safe' list.

How Long Will It Take Effect?

Typically, increases in breast milk production by taking fenugreek capsules occur within 24 hours, but may sometimes extend 2-3 days after initial supplementation, with full effectiveness attained within 2 weeks. Once milk flow has been successfully established, fenugreek usage can be stopped without a corresponding interruption in breast milk flow.

Side Effects

The most common side effect is a maple syrup odour in urine and sweat (this is not a bad thing; it just means that the proper dosage has been reached). In overdose, fenugreek can cause diarrhoea, aggravate asthma, or induce hypoglycaemia.

Precautions

Mothers who suffer from asthma, or who are allergic to peanuts, should probably avoid taking fenugreek, unless otherwise indicated by your doctor. Pregnant women should avoid taking fenugreek because it can cause uterine (womb) contractions (i.e. induce labour).

Makanan Pejal: Tunggu!! Dia BELUM Bersedia.

"Kau dulu, umur dua bulan mak dah bagi pisang bakar, bubur nasi..elok je kau membesar." "Cuba bagi air kanji kat anak tu, mesti lepas ni tidur lena"

Those dialogues sound familiar right? Itu antara perbualan ibu-ibu 'dulu' yang merujuk kepada situasi dan pengalaman mereka pada tahun 80-an. Waktu itu strategi pemasaran bagi susu formula dan makanan bayi dalam tin atau jar betul-betul agresif! Sehingga membuatkan orang seolah-olah sudah tidak percaya kepada kebaikan penyusuan susu ibu secara eksklusif.

Yang lebih merisaukan, pada zaman di mana maklumat berada di hujung jari, masih ramai ibu-ibu 'baru' yang percaya bayi sepatutnye diberikan makanan pejal lebih awal. Ibu-ibu ini percaya bahawa susu mereka sudah tidak memadai untuk mengenyangkan dan membuatkan bayi ini membesar dengan sihat.

Ada beberapa tanda yang membuatkan kita 'merasakan' bayi kita sudah boleh mula diberikan makanan pejal, walaupun dia belum pon mencecah usia 6 bulan (seperti saranan WHO).

1. Berjaga di waktu malam
Kesukaran bayi untuk tidur lena di waktu malam membuatkan ada di antara kita fikir yang bayi itu lapar. Maka dengan memberikan makanan pejal, diharapkan bayi itu akan kenyang dan akan tidur sepanjang malam tanpa perlu bangun untuk disusukan.
Sebenarnya, bayi di bawah usia enam bulan sekiranya disusukan setiap kali ada permintaan (feed on demand), ia sudah memadai dan terbaik untuk membuatkan bayi kenyang. Bukan kah bayi juga manusia seperti kita yang punyai perlbagai ragam? Banyak lagi sebab-sebab lain yang membuatkan bayi sukar tidur di waktu malam.

2. Berat badan tidak naik
Kita biasanya akan mudah panik bila berat bayi tidak ada peningkatan seperti yang dijangkakan. Lebih-lebih lagi bila graf tumbesaran sudah mula mencecah garisan kuning. Atas nasihat orang sekeliling, bayi yang baru berusia tiga bulan sudah diberikan bijirin bayi dalam tin supaya bayinya akan lebih besar dan gemuk serta 'comel & montel' seperti anak-anak orang lain.
Hakikatnya, kajian menunjukkan adalah normal (khususnya bagi bayi yang diberikan susu ibu) apabila berat badan agak perlahan peningkatannya ketika di usia empat bulan (blh rujuk sini).

3. Tidak puas menyusu
Tiba-tiba bayi kelihatan tak puas disusukan. Bayi mungkin minta disusukan hampir setiap jam atau kurang daripada sejam, atau bekalan EBM yang dihantar kepada pengasuh tiba-tiba jadi tak cukup.
Tahukah anda, itulah namanya growth spurt. Biasanya berlaku ketika usia bayi 2-3 minggu, 4-6 minggu, 3 bulan dan 4 bulan (bagi kes bayi kurang dari 6 bulan). Bukan tandanya bayi itu sudah boleh mula diberikan makanan pejal. Teruskan penyusuan mengikut permintaan bayi. Itu sudah betul-betul cukup untuk keperluan tumbesaran bagi bayi di bawah usia 6 bulan.

4. Meniru aksi mengunyah
Bermula lebih kurang di usia 4 bulan, melihat aksi ahli keluarga dan orang sekeliling adalah sesuatu yang menakjubkan bagi mana-mana bayi sekali pun. Meniru aksi-aksi kita adalah sesuatu yang normal, termasuklah meniru aksi kite mengunyah atau menggerak-gerakkan bibir. Tapi itu bukanlah bermakna bayi ini sudah wajar diberi makanan pejal.

5. Tidak tidur selepas disusukan
Keadaan ini membuatkan ibubapa atau penjaga/pengasuh menganggap susu yang diberikan tidak lagi mencukupi untuk menidurkan bayi.
Tapi sedarkah kita tatkala usia bayi semakin meningkat (lebih kurang 4 bulan), bayi sebenarnya sudah semakin alert dan semakin teruja memberi reaksi kepada keadaan sekeliling? Sebab itu semakin ia membesar, semakin kurang waktu tidurnya!

6. Saiz bayi yang besar
Bayi yang bersaiz lebih besar berbanding bayi-bayi lain yang seusianya tidak bermakna sistem penghadaman dan pencernaan serta sistem imunitinya sudah lebih matang! Mungkin bayi ini besar kerana faktor genetik, atau kesan dari growth spurt, atau over-feeding bagi bayi yang dberikan susu formula. Maka bayi bersaiz besar tidak bermakna dia sudah memerlukan makanan pejal untuk menampung tumbesarannya yang extra itu.

Itu antara false sign terhadap kesediaan bayi kepada makanan pejal. Maka, buatlah pemerhatian dan keputusan yang bijak. Avoid to quickly jump into a conclusion. Make an informed decision. :)

Wakil dari kedua-dua kumpulan (Ms Aqilah & Ms Nafizah) membentangkan point-point yang hampir sama. Maka saya buat kesimpulan dari perbincangan di kedua-dua kumpulan.

Memberi makanan pejal (atau solid food) kepada bayi adalah antara moment yang dinanti-nantikan oleh ramai ibu. Pengenalan kepada makanan pejal menunjukkan bayi anda sudah melangkah setapak lagi ke hadapan, yeay!! Ibu bapa akan menjadi sangat teruja menantikan saat ini. Siaplah bapanye dengan DSLR di tangan, stand-by untuk capture pose-pose comel dan menggelikan hati dan wajah asyik menikmati makanan pejal.
Tapi bilakah waktu yang paling sesuai untuk mula memperkenalkan makanan pejal kepada bayi anda?
Mengikut WHO, 6 bulan adalah waktu yang sesuai anda memperkenalkan makanan pejal.

Kenapa harus mengunggu sehingga 6 bulan?
Di usia sebegitu, organ-organ dalaman bayi sudah bersedia untuk menerima dan memproses selain dari susu ibu. Tetapi ada juga bayi sudah mula diberikan makanan pejal seawal usia 5 bulan setengah, dan tidak terkejut juga, ada yang hanya menerima makanan pejal ketika di usia 7 bulan.

Petikan dari laporan kajian WHO, 2011, bayi yang diberikan makanan pejal sebelum usia 6 bulan akan memberikan kesan negatif kepada kesihatan ibu dan bayi:
Bayi lebih terdedah kepada risiko jangkitan gastrointestinal dan cirit-birit (Kramer, 2003)
Ibu tidak mengalami penurunan berat badan yg optimum dan kitaran haid akan datang lebih awal

Bagaimana kalau saya ingin meneruskan penyusuan eksklusif tanpa makanan pejal walaupun bayi saya sudah berusia lebih dari 6 bulan?
Melewatkan pengenalan kepada makanan pejal pula mugkin menyebabkan bayi anda kekurangan zat besi.

Apakah tanda-tanda yang bayi anda sudah bersedia untuk menerima makanan pejal?

bayi mampu duduk dengan tegak
tounge-thrust reflex sudah berkurangan; iaitu bayi sudah mampu koordinat mulut dan lidahnya untuk kunyah & telan
bayi sudah mula mengutip objek di hadapannya dan memasukkan ke dalam mulut (sounds familiar..?)
bayi sudah menunjukkan minat terhadap makanan anda dengan cara ingin menerkam makanan anda :)

Sebagai seorang ibu, biasanya anda akan dapat 'baca' akan kewujudan tanda-tanda bayi anda sudah boleh mula diberikan makanan pejal.

Wahh!! Anak saya sudah ada tanda-tanda tersebut. Nak mula dengan makanan apa ya? Ne**um? Ru*k? H**nz?

Natural is always better.

Jika dirujuk kepada Kementerian Kesihatan Malaysia, bayi bersusia 6 bulan boleh mula diberikan makanan berbentuk puri, iaitu dikisar atau dilecek halus. Bagi 8 bulan ke atas, memadai makanan itu ditapis atau dikisar kasar.

Menyediakan makanan pejal dengan air tangan ibu sendiri (atau homecooked baby food) amatlah digalakkan. Lebih segar, kebersihan yang terjamin, tanpa penstabil, tanpa gula, garam dan perisa tambahan dan ibu bebas untuk berkreativiti dengan resepi-resepi makanan pejal yang perlbagai! Ingat: No salt, No sugar. Perasakan puri atau bubur bayi anda dengan perasa asli. Contoh: air rebusan ayam, air rebusan daging, ubi kentang (rasa lemak), carrot dan jagung (rasa manis), ikan bilis digoreng dan dikisar halus, hirisan bawang putih, bawang merah dan bawang besar. Semua ini sudah cukup untuk membuka selera bayi anda! Silap-silap, mummy dan daddy pon join sama.

Puri berasaskan beras, buah-buahan dan sayur-sayuran sesuai diberikan sebagai permulaan. Amalkan pemerhatian selama 4 hari atau 4 Days Rule. Sekiranya bayi anda menunujukkan tanda-tanda alahan (cth: ruam pada kulit, sesak nafas), itu tandanya makanan tersebut tidak sesuai untuknya di usia tersebut. Sekiranya bayi tidak menunjukkan sebarang tanda negatif sepanjang 4 hari tersebut, maka bolehlah anda bereksperimen dengan campuran puri-puri berbeza mengikut kreativiti anda.

Sebanyak mana harus saya berikan?
Untuk permulaan, cukup 1-3 sudu untuk satu hidangan, dua kali sehari.
Berkemungkinan bayi anda akan menunjukkan muka 'pelik' ketika mula diperkenalkan kepada makanan pejal. Usah bimbang. Dan usah dipaksa. Cuba lagi di waktu lain.

Tips: Untuk mengurangkan kejutan bagi bayi yang diberikan susu ibu secara eksklusif, campurkan puri pertama untuk bayi anda dengan sedikit susu perahan anda. Rasa manis pada susu ibu biasanya akan membantu bayi menerima makanan pejal.

Anak saya sungguh berselara. Bolehkah saya menambah kuantiti dan kekerapan memberikan makanan pejal?
Anda lebih memahami bayi anda! Tidak salah memberikan makanan pejal mengikut permintaan bayi anda. Tapi harus diingat, bagi bayi berusia kurang dari satu tahun, susu ibu masih merupakan makanan utama mereka. Pemberian makanan pejal tidak seharusnya menggantikan penyusuan susu ibu.

Saya ibu bekerja. Bagaimana caranya untuk saya sediakan makanan pejal untuk bayi saya setiap hari?
Dengan pengendalian yang hygienic, sumber bahan mentah yang segar, bubur atau puri boleh disediakan dalam kuantiti yang banyak. Setelah puri disejukkan, bahagikan puri dalam kuantiti kecil (untuk satu hidangan) dan terus bekukan. Apabila memerlukan, hanya keluarkan kuantiti yang diperlukan. Nyah-beku dan suamkan sebelum dihidangkan kepada bayi anda. Memang praktikal!

Pernah dengar tentang Baby-Led Weaning ? Di FTT ini, ibu-ibu juga turut berkongsi mengenai topik ini. Selain dari memberi makanan pejal mengikut garis panduan yang dikeluarkan oleh Kementerian Kesihatan Malaysia, kaedah ini boleh dicuba.

Kenapa Baby-Led Weaning?
Bayi anda yang akan menentukan bila dia hendak makan, sebanyak mana yang dia mahu makan, makanan mana yang akan dimakan lebih banyak dan bila dia akan berhenti. Menarik kan?

Kaedah ini juga memberi peluang untuk si comel 'bermain' dengan tekstur, rasa dan warna makanan. Disebalik situasi yang sememang nya comot ini, bayi anda sebenarnya akan belajar menggunakan soft motor skill dan koordinasi di antara mata, tangan dan mulut. Cara ini disifatkan lebih alami, sebab secara fitrahnya, manusia akan memasukkan makanan ke dalam mulut, kunyah dan telan. Peringkat mengunyah hampir tak wujud kalau kita sekadar memberikan puri kepada bayi.

Anak saya belum ada gigi. Sesuaikah kaedah ini?
Di usia 6 bulan, gusi bayi sebenarnya sudah mengeras dan bersedia untuk mengunyah. Kalau tak percaya, cuba masukkan jari kelingking anda yang bersih dan biarkan dia gigit. Haa, waktu itu anda akan tahu betapa kuatnya gigitan bayi anda!

Err, tak tercekik ke diberi makanan yang tidak dilecek.
Mengikut kajian, bayi di usia 6 bulan ke atas sebenarnya sudah ada mekanisma unik iaitu bayi hanya akan menolak makanan masuk makanan yang dia sudah kunyah dan mampu telan, dan akan menolak keluar sekiranya dia tak mampu untuk telan dengan cara memuntahkannya atau batuk.

Peraturan Baby-Led Weaning:

Makanan diberikan tanpa penggunaan sudu dan mangkuk/pinggan.
Ibu mesti berada di samping bayi sepanjang dia makan.
Aktiviti ini memang comot, maka bersedia dari segi mental dan fizikal for all the mess.

Antara makanan yang boleh dihidangkan untuk kaedah Baby-Led Weaning ialah:

Pisang
Labu yang direbus
Lobak merah yang direbus/distim
Jagung rebus
Buah-buahan lain
Mi dan pasta

Kaedah ini dikatakan akan membuatkan bayi anda terdedah kepada kepelbagaian makanan dan akan lebih 'menghargai' makanan.

Sebelum mengakhiri sesi FTT kali ini, Dr Ernie mengingatkan, Baby-Led Weaning bukanlah satu polisi dari WHO dan Kementerian Kesihatan Malaysia. Cuma ibu bapa mempunyai pilihan tambahan selain dari mengikut garis panduan yang dikeluarkan oleh KKM. Dr Ernie juga menekankan peraturan 4 Days Rule yang harus dipraktikkan terutama di awal pengenalan terhadap makanan pejal. Tujuannya adalah untuk melihat reaksi negatif atau alahan sekiranya ada.

Di akhir perbincangan, ibu-ibu nampaknya lebih bersemangat dan bersedia untuk menyediakan sendiri makanan pejal untuk bayi mereka..siap salin resepi dan kaedah penyediaan lagi. Mungkin lain kali boleh diadakan demo masakan dan penyediaan makanan pejal untuk bayi. Ada yang berminat untuk jadi chef? :)
